care options, and make informed choices.
Pathways Social Workers provide counseling and consultation,
information and referral to community resources, and, when
funding allows, limited care management for needy clients.
We know that every person is an individual, with specific
strengths and challenges. So we begin by listening carefully
and honoring each unique situation.
Special care is taken to address finances, medical issues,
support systems, housing, and spiritual concerns for seniors
and their families. In addition, caregiver issues are strongly
considered and options are developed.
Pathways also provides both Medicare-approved and private Care Management Services to help clients and their families define a comprehensive care plan, manage activities across different providers, and coordinate ongoing delivery of all client services. Social Worker from Pathways can make your life simpler.
Personalized, confidential service can be provided by telephone,
at the offices of the Foundation for Senior Living, or in
some cases in your home, Monday through Friday from 8 am
to 5 pm.
