Summary of Services

Adult Day Health Services – Centers throughout Maricopa County providing medical, recreational, and social services in a safe, caring environment for seniors and young adults with disabilities.

Adult Foster Care – Recruitment, training, and monitoring of sponsors who integrate seniors and adults with disabilities into their households.

Assisted Group Living
– Home-like settings that provide supervision, supportive personal care, and therapeutic services to adults with mental illness.

Assisted Living Education – Arizona Department of Health Services approved training program for Certified Caregivers and Managers pursuing a career working in Assisted Living Facilities. Classes also available for CEUs and Family Caregivers.

Care By Design
– Providing customized in-home services such as companionship, transportation, respite, home care, personal care, care management and caregiver support on a fee-for-service basis.

Community Action Programs – Emergency services - food, utilities, rent, referral services, and short-term case management.

Home Care – Enabling individuals to remain independent in their homes, with the help of housekeeping, chore, personal care and respite services. In rural counties, provides visiting nurse and case management services.

Home Health Care
– A licensed, certified Medicare Home Health Agency providing skilled nursing and home health aide services throughout Maricopa County.

Home Energy Solutions – providing expertise and support for local utilities and training for Weatherization and ENERGY STAR contractors throughout the Southwest.

Home Improvements
– Minor, major and emergency home repairs and weatherization services for low-income individuals, seniors, and adults with disabilities.

Housing – Affordable housing throughout Arizona providing individual apartments for low-income seniors and adults with disabilities.

Pathways – Social workers helping seniors, adults with disabilities and their families access community resources and care options.

Senior Centers
– Providing social activities, congregate lunch program, home-delivered meals, health education and referral services for seniors in Peoria and Wickenburg.

The Caring Place – Information and referral, counseling, care management, personal and health services and spiritual support in the Northeast Valley.

The Catholic Outreach Center in the Sun Cities – Providing counseling and family support, care management, spiritual support and educational programs in the Sun Cities area.

The FSL Caregiver House - A training center, demonstration lab and information and referral site for family and paid caregivers as well as seniors and adults with disabilities who actively manage their own care.

Young Adult Day Health Care – Young adults, ages 18-45, who have a closed head injury or other disability, enjoy a club-like environment and access to restorative therapies.
