By Design – Providing customized in-home services such as companionship,

apartments for low-income seniors and adults with disabilities.
Pathways – Social workers helping seniors, adults with disabilities and their families access
community resources and care options.
Senior Centers – Providing social activities, congregate lunch program, home-delivered
meals, health education and referral services for seniors
in Peoria and Wickenburg.
The Caring Place – Information and referral, counseling, care management,
personal and health services and spiritual support

Catholic Outreach Center in the Sun Cities – Providing counseling
